Molina Healthcare is a healthcare company committed to providing quality services to its customers. We are currently seeking a skilled Senior IRIS Program Coordinator to join our team.

Job Summary

We are looking for a highly motivated and organized individual with excellent communication skills to coordinate the IRIS program for our participants. As a Senior IRIS Program Coordinator, you will be responsible for building relationships with participants, identifying their long-term care goals, and helping them navigate the Wisconsin IRIS program.

About the Role

This is a remote position, where you will partner with people in your community who are enrolled in the Wisconsin IRIS Program and the Molina Healthcare IRIS Consultant Agency. You will have regularly scheduled visits with IRIS participants in their home and community, and be available upon request to support their needs.

Key Responsibilities

  • Provide program orientation to new participants, explaining their rights and responsibilities as someone enrolled in the IRIS program
  • Build relationships with participants, understanding their goals, interests, and connections with the local community
  • Assist participants in identifying personal outcomes and ensuring those outcomes are being met, while staying within their IRIS budget
  • Document all orientation and planning activities within the IRIS data system within 48 business hours of the visit
  • Research community resources and natural supports that fit each participant's individual outcomes, sharing that information as it becomes available
  • Educate participants on how to read and interpret their monthly budget reports, ensuring they operate within their budget
  • Liaise between the Fiscal Employer Agency and the IRIS Consultant Agency, assisting participants with provider billing, tracking receipts, and ensuring workers are paid

Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a social work, psychology, human services, counseling, nursing, special education, or closely related field (or four years of commensurate experience)
  • 1+ year of direct experience related to delivering social services to individuals with intellectual or physical disabilities and older adults
  • Excellent problem-solving skills, critical thinking skills, and strong basic math skills
  • Ability to work independently, with minimal supervision, and be self-motivated
  • Comfortable working within a variety of settings, adjusting style as needed, and working with a diverse population
