About the Role

As a Health Education Coordinator for Preventive Services, you will play a critical role in promoting healthy behaviors and reducing health disparities among vulnerable populations. Our team is dedicated to delivering high-quality, patient-centered care, and we're seeking a motivated and compassionate individual to join our ranks.

The estimated salary for this role is $**65,000 - $80,000 per year**, depending on experience and qualifications. Key Responsibilities

  • Design and deliver evidence-based health education programs for individuals and groups, focusing on HIV/STI prevention, PrEP navigation, and harm reduction.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure comprehensive care coordination and linkages to needed services.
  • Conduct regular program evaluations and make data-driven decisions to improve program effectiveness.
  • Develop and maintain relationships with community partners to promote collaborative efforts and resource sharing.

Requirements

  • Bachelor's degree in a related field (health education, public health, social work, etc.).
  • Experience in health education, case management, or community outreach preferred.
  • Fluency in English and Spanish required; bilingualism in other languages a plus.
  • Strong analytical and problem-solving skills, with ability to collect and analyze data to inform program improvements.
