Prevention Health Specialist

About the Role:

We are seeking a highly motivated Prevention Health Specialist to join our team at Open Door Family Medical Center. As a key member of our healthcare team, you will play a crucial role in providing high-quality care and support to our patients.

Key Responsibilities:
  • Identify and engage with potential candidates for comprehensive sexual health education and PrEP/PEP navigation services using data reports and outreach activities.
  • Provide education on prevention topics, including HIV/STI/HCV/PrEP/PEP, condom use, safer sex practices, harm reduction, regular STI and HIV testing, and mental health and substance use issues related to HIV risk.
  • Refer individuals for HIV/STI/HCV testing and collaborate with medical providers to coordinate follow-up after reactive/positive results, including navigation to medical care and assistance with partner notification.
Requirements:
  • Bachelor's degree in an applicable field or equivalent experience.
  • Fluent in English and Spanish.
  • Strong interpersonal skills, including active listening and clear oral communication.
