Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a compassionate and skilled Private Duty Nurse LPN to provide high-quality care to our medically fragile patients in the comfort of their own homes.

Job Summary:

We are looking for a dedicated and experienced LPN to join our team of healthcare professionals. As a Private Duty Nurse LPN, you will be responsible for providing one-on-one care to our patients, implementing physician-prescribed plans of care, and coordinating with our team to ensure seamless care delivery.

Responsibilities:
  • Implement physician-prescribed plans of care for our patients
  • Conduct patient assessments and coordinate care with our team
  • Administer prescribed medication, treatments, and therapies as needed
  • Document care provided in our system of record
  • Provide health education and training to family members
  • Accept and write verbal orders from physicians as required
  • Follow up with physicians, clients, family members, and staff on patient problems, concerns, and changes in condition
Requirements:
  • Active, unencumbered LPN license in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
  • Reliable transportation
