Position Summary:
We are actively seeking a full-time Nurse Practitioner specializing in Post-Acute and Long-Term Care to serve at our affiliated Nursing and Rehabilitation Centers. This role is essential in ensuring that our patients receive the highest standard of care.
Key Features of the Role:
- Full-time schedule with flexible hours, Monday through Friday
- Comprehensive training and quality assurance feedback for our medical staff
- Access to necessary tools and processes to prioritize patient care
Responsibilities:
- Conduct thorough examinations of patients and review their medical histories
- Order and interpret diagnostic tests, including blood tests and imaging studies
- Diagnose health conditions and propose appropriate treatment plans
- Prescribe medications and monitor patient responses
- Maintain accurate medical records and patient schedules
- Educate patients and their families about health conditions and treatment options
- Promote preventive health practices and overall wellness
- Engage in continuous professional development and research initiatives
Qualifications:
- Demonstrated experience as a Nurse Practitioner in a skilled nursing setting
- Proficient in primary, acute, and chronic care practices
- Familiarity with healthcare regulations and patient care standards
- Exceptional communication and interpersonal skills
- Strong ethical standards and a collaborative spirit
- Commitment to ongoing learning and clinical excellence
- Valid Nurse Practitioner license required
- Master's degree; PhD is advantageous
