We are actively seeking a full-time Nurse Practitioner specializing in Post-Acute and Long-Term Care to deliver services at our partner Nursing and Rehabilitation Centers.
This position offers:
- A full-time schedule with flexible hours, Monday through Friday.
- A supportive environment featuring comprehensive training and quality assurance feedback for our medical staff.
- Access to essential tools and processes that allow you to concentrate on patient care.
As part of a collaborative team, including the Medical Director and Attending Physician, you will be responsible for admitting, rounding on, and discharging patients in short-term rehabilitation and long-term care settings. Your duties will include conducting history and physical examinations, performing medical rounds, and providing cross-coverage as necessary. You will evaluate patients experiencing changes in their condition and attend to requests from facility staff while ensuring compliance with all quality initiatives.
Key Responsibilities
- Conduct thorough examinations of patients and review their medical histories.
- Order and analyze diagnostic tests, such as blood tests and X-rays.
- Diagnose various health conditions and illnesses.
- Develop treatment plans for chronic and infectious diseases.
- Prescribe necessary medications.
- Maintain accurate patient records and schedules.
- Communicate health conditions and prescribed treatments to patients.
- Educate patients and their families on disease prevention and management.
- Oversee the overall health of patients.
- Promote healthy lifestyle practices.
- Engage in ongoing professional development and research.
Required Skills
- Demonstrated experience as a Nurse Practitioner in a skilled nursing context.
- Expertise in primary, acute, and chronic care practices.
- Familiarity with health regulations and patient care standards.
- Excellent communication abilities.
- Strong ethical standards.
- Ability to work effectively within a team and maintain a positive attitude.
- Patience and composure in various situations.
- Commitment to continuous learning and clinical development.
- Valid Nurse Practitioner license.
- Master's degree; a PhD is advantageous.
