Staff Nurse

As a Staff Nurse at Kaiser Permanente, you will provide high-quality nursing care to a defined group of patients on an on-call basis. You will work collaboratively with the healthcare team to coordinate and facilitate healthcare services for patients, adhering to Kaiser Foundation Health Plan of Washington ambulatory Nursing Care Standards and the Nursing Code of Ethics.
Essential Responsibilities:- Conduct ongoing assessments of patient care needs, using nursing diagnosis to describe this assessment and develop a written plan of care.
- Assist patients and families/significant others in developing health care goals, which are part of the individualized plan of care.
- Provide nursing interventions safely and competently, providing the patient and family/significant others with information preceding interventions.
- Provide telephone consultation to patients and family/significant others, providing ongoing evaluation of the patient's response to care.
- Anticipate and recognize changes in the patient's status and need for nursing care, modifying the individual plan of care in collaboration with the healthcare team.
- Collaborate, communicate, and coordinate with other healthcare team members in planning and implementing care.
- Assess the needs of designated patients requiring hospitalization, initiating nursing admission database and discharge planning in collaboration with inpatient staff and physician.
- Communicate recognition of the patient's and family/significant others' psychosocial, cultural, and emotional needs in each nurse-patient interaction.
- Identify the patient's educational needs as part of the nursing assessment, developing an educational component for each nursing care plan.
- Provide teaching/counseling congruent with the patient's and family/significant others' learning needs and the medical plan of care.
- Assess patient lifestyle risk factors, integrating health promotion and preventive aspects of care with the ongoing assessment of nursing care needs.
- Work with patients to develop a plan to improve general health status, implementing physical and psychological preventive measures.
- Adhere to KFHPW documentation standards, providing a safe environment for each patient and adhering to KFHPW safety standards.
- Adhere to KFHPW infection control standards and guidelines, including the guidelines for reporting communicable diseases.
