Primary Care Specialist

Molina Healthcare is seeking a highly skilled and compassionate Primary Care Specialist to join our team of dedicated healthcare professionals. As a Nurse Practitioner, you will play a vital role in delivering high-quality patient care in various community settings, including homes, nursing facilities, and public locations.
Job SummaryThe ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for providing patient-centered care. You will work collaboratively with our team to develop and implement effective treatment plans, ensuring that patients receive comprehensive and coordinated care.
Responsibilities- Provide primary care services to patients in various community settings, including home visits, nursing facilities, and public locations.
- Conduct thorough medical assessments, order diagnostic tests as needed, and develop treatment plans tailored to each patient's unique needs.
- Establish and maintain relationships with patients, families, and healthcare providers to ensure seamless coordination of care.
- Collaborate with other healthcare professionals to develop best practices and improve patient outcomes.
- Stay up-to-date with the latest clinical guidelines, regulations, and industry developments to provide evidence-based care.
- Prioritize patient safety, confidentiality, and well-being at all times.
- A Master's degree in Family Health from an accredited nursing program.
- An active national certification from one of the following organizations: American Academy of Nurse Practitioners or American Nurses Credentialing Center.
- A current state-issued license to practice as a Family Nurse Practitioner.
- Current Basic Life Support for Healthcare Professionals certification.
- Excellent communication, interpersonal, and problem-solving skills.
- A minimum of 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, preferably in a community health setting.
