Homebound Palliative Care Nurse Practitioner

We are seeking an experienced Nurse Practitioner to join our team as a Homebound Palliative Care Nurse Practitioner - Primary Care Specialist. This role will involve providing high-quality care to patients in their homes, working closely with our clinical team to deliver exceptional patient outcomes.
Key Responsibilities- Assess and manage the health needs of homebound patients, including those with chronic diseases and palliative care requirements.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote self-management and independence.
- Maintain accurate and up-to-date records of patient interactions, including progress notes and treatment plans.
- Bachelor's degree in Nursing or related field; Master's degree preferred.
- Current licensure as a Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN); certification as a Nurse Practitioner (NP) required.
- Minimum 1-2 years of experience in primary care or palliative care; experience working with geriatric populations highly desired.
- Strong clinical skills, including assessment, diagnosis, and treatment planning.
- Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
- Competitive salary range: $140,000 - $160,000 per year, depending on experience.
