Job Summary:


We are seeking an experienced RN/LPN Nurse Coordinator to join our team in a dynamic and supportive environment. As a Medical Care Coordinator, you will play a crucial role in providing integrated, person-centered care to our patients, addressing their complex physical and behavioral health needs.


About the Role:


In this position, you will be responsible for evaluating patient needs, assessing their physical, environmental, and psycho-social health issues, and working collaboratively with our multi-disciplinary team to develop and implement effective care plans. You will participate in the selection of strategies to implement evidence-based wellness and prevention initiatives, communicate with medical providers and subspecialty providers, and provide education on health conditions, treatment recommendations, medications, and strategies to implement care plan goals.


Key Responsibilities:



  • Evaluate patient needs and requirements to achieve optimal wellness state
  • Assess patient's physical, environmental, and psycho-social health issues and work in collaboration with a multi-disciplinary team
  • Participate in initial care plan development including specific goals for all enrollees
  • Communicate with medical providers, subspecialty providers, and other healthcare professionals regarding records and patient care
  • Provide education on health conditions, treatment recommendations, medications, and strategies to implement care plan goals
  • Monitor assessments and screenings to assure findings are integrated in the care plan
  • Facilitate the use of electronic health records (EHRs) and other healthcare information technology (HIT) to link services, facilitate communication among team members, and provide feedback
  • Monitor and report performance measures and outcomes
  • Meet regularly with the care team to plan care and discuss cases

Requirements:


To be successful in this role, you will require:



  • An active LPN or RN license
  • Knowledge and ability to use computer software, including Microsoft Office and database management
  • Experience in addiction and needs identification
  • Able to communicate effectively, both orally and written
  • Able to work independently and as part of a team
  • Creative problem solver with ability to suggest ways to enhance program
  • Ability to plan, organize, set objectives, time frames, and prioritize

Benefits:


This position offers a competitive salary range of $65,000 - $85,000 per annum, commensurate with experience, and a comprehensive benefits package, including medical, dental, and vision insurance, paid time off, and professional development opportunities.
