About the Role

We are seeking a compassionate and skilled Nurse Practitioner (APRN) to join our team at Abode Care Partners, a leading provider of integrated medical services.

As a Senior Medical Professional, you will be responsible for providing evidence-based medical care to seniors and patients with complex needs in various settings. Your expertise will play a critical role in enhancing patient quality of life.

Responsibilities:
  • Perform medical assessments, develop, and implement comprehensive care plans tailored to individual patient needs.
  • Collaborate with healthcare professionals to ensure seamless interdisciplinary care and maintain accurate patient records.
  • Order and interpret laboratory and diagnostic testing as needed to inform treatment decisions.
Qualifications:
  • Possess an active state Nurse Practitioners license.
  • Hold current Nurse Practitioner National Board Certification.
  • Hold active state DEA licensure.
  • Have a Master's degree in Nursing (MSN).
  • Accumulate at least five years of nursing experience, with a minimum of two years of nurse practitioner experience preferred.
  • Exhibit strong interpersonal communication skills – verbal and written.
  • Demonstrate excellent time management skills with accountability for daily workflow.

Salary Range:

The estimated annual salary range for this position is $100,000 - $120,000, with total earnings potential reaching up to $130,000+ based on performance.
