Job Summary:

Mental Health Partners is seeking a skilled Housing Case Manager to join our team. As a Housing Case Manager, you will play a critical role in providing supportive services to clients living at our partnership location, the Suites, and managing client care for the transitional housing program at the Inn Between. This position requires a strong ability to work collaboratively with clients, community partners, and other professionals to provide comprehensive case management services.

Key Responsibilities:
  • Provide supportive services and case management to clients living at the Suites and participating in the transitional housing program at the Inn Between.
  • Manage grant paperwork, enter HMIS documentation, and develop and maintain partnerships with community organizations.
  • Collaborate with Mental Health Partners teams to provide comprehensive services to clients.
  • Develop and maintain strong relationships with clients, community partners, and other professionals to ensure seamless service delivery.

Requirements:
  • Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor's degree.
  • Experience working with adults and/or children in an outpatient setting.
  • Current CO Driver's License and a safe driving record.
  • Ability to work in a fast-paced environment and adapt to changing client needs.
