We're seeking a skilled RN Case Manager to support our diverse member population with education, advocacy, and connections to the resources they need to feel better and get well. As a key member of our team, you'll make outbound calls and receive inbound calls to assess members' current health status, identify gaps or barriers in treatment plans, and provide patient education to assist with self-management. You'll also make referrals to outside sources and provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction. This is a high-volume, customer-service environment that requires efficiency, productivity, and thoroughness when dealing with our members over the phone. Strong computer and software navigation skills are critical, as well as a strong patient-focus and adaptability to changes. Required qualifications include a current, unrestricted RN license in the state of employment, 3+ years of experience in a hospital, acute care, or direct care setting, and the ability to type and navigate a Windows-based environment. Preferred qualifications include a BSN, a background in managed care, case management experience, and certification as a Case Manager (CCM). **Key Responsibilities:**
