Optum Home & Community Care is seeking a skilled and compassionate Clinical Specialist to join our HouseCalls team. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities- Conduct In-Home Assessments: Perform comprehensive physical exams, evaluate past medical history, review systems, and reconcile medications to identify potential health risks and gaps in care.
- Screenings and Testing: Conduct evidence-based practice screenings, including point-of-care testing, to inform care management and active medical management.
- Care Coordination: Communicate findings with patients' primary care physicians to ensure seamless care transitions and address potential gaps in care.
- Patient Education: Educate patients on disease processes, medication management, and compliance to empower them to take an active role in their health.
- Urgent and Emergent Situations: Identify and intervene in urgent and emergent situations, ensuring patients receive timely and appropriate care.
- Active NP or PA License: Possess an active NP or PA license or be able to obtain one by start date, with no encumbrances or disciplinary actions.
- Certification: Hold active national NP or PA certification or be able to obtain it in the state of assignment by start date.
- Physical Requirements: Meet the physical demands of the job, including lifting a 30-pound bag, navigating stairs, and performing physical assessments.
- Reliable Transportation: Possess reliable transportation to complete home visit assessments.
- Experience: Possess 1+ years of clinical experience in their highest level of education, clinical setting.
- Additional Licensure: May be requested to obtain additional licensure in other geographic areas.
- Specialized Experience: Possess experience in gerontology, cardiology, internal medicine, or endocrinology, or home health care or home visit experience.
