Optum is seeking a skilled NP or PA to join our HouseCalls team in Oregon. As a clinician-led care organization, we're dedicated to creating a seamless health journey for patients across the care continuum.

In this role, you'll provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks. You'll work closely with our integrated medical teams to coordinate follow-up care and improve patient outcomes.

Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate
  • Identify diagnoses to inform care management and active medical management
  • Communicate findings to inform members' primary care physicians of potential gaps in care
  • Intervene in urgent and emergent situations
  • Educate members on disease processes, medication, and compliance

Requirements:

  • Active NP or PA license or ability to obtain by start date
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
  • Reliable transportation for home visit assessments

Preferred Qualifications:

  • 1+ years of clinical experience in a related setting
  • Ability to travel throughout the state, up to 5% of the time, with advanced notice
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
