Job Summary:

We are seeking a skilled LPN to join our Primary Care team in Tualatin, Oregon. As a key member of our healthcare team, you will provide high-quality patient care and support to our clinicians and other healthcare professionals.

Key Responsibilities:
  • Clinical Tasks:
    • Prepare patients for office visits and procedures, ensuring all necessary tests and information are ready for review.
    • Perform focused nursing assessments and triage patients with routine or self-limiting medical conditions.
    • Recognize signs and symptoms of common medical conditions and implement planned interventions as delegated by clinicians.
    • Contribute to the development and implementation of patient care plans under the direction of RNs/clinicians.
  • Clinical Procedures:
    • Perform diagnostic tests, therapies, and procedures within the scope of practice, maintaining proficiency and adhering to established policies and procedures.
    • Administer medications via various routes, including oral, topical, and intravenous.
    • Provide wound care, including routine and complex dressing changes.
    • Assist with medical procedures, such as biopsies and joint aspirations.
  • Practice Management Activities:
    • Maintain and adjust patient schedules to ensure efficient and effective care.
    • Respond to incoming messages and requests, collecting and communicating pertinent data for clinician review.
    • Facilitate follow-up duties on patient care plans, including processing prescriptions and forms, and scheduling procedures and tests.
Requirements:
  • Graduate of an accredited school of nursing.
  • Practical Nurse License (Washington) within 6 months of hire.
  • Basic Life Support certification.
  • Demonstrated clinical nursing skills, interpersonal and customer service skills, and communication skills.
Preferred Qualifications:
  • Two years of nursing experience in an ambulatory care setting.
  • IV therapy experience or recent certification.
  • Familiarity with documenting in an electronic medical record system.
