RN Case Manager

HealthEcareers - Client is seeking a skilled RN Case Manager to join our team in New Mexico. As a RN Case Manager, you will conduct in-home nursing visits for our clients, providing physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers.
Key Responsibilities:- Provide direct care and case management to assigned clients
- Assist and collaborate with regional directors and other personnel to identify and correct issues
- Plan, implement, and evaluate care provided
- Participate in client care conferences and serve as a local on-site clinical resource
- Adhere to nursing delegation guidelines and Agency policies
- Graduate of a state-approved school of professional registered nursing
- BSN preferred
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
