Professional Case Management is seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing high-quality care and support to our clients, ensuring their needs are met and their lives are improved.
Key Responsibilities
- Conduct in-home nursing visits to assess clients' needs and provide care and support
- Develop and implement individualized care plans to meet clients' unique needs
- Collaborate with healthcare professionals and family members to ensure comprehensive care
- Provide education and support to clients and their families on health-related topics
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements
- Graduate of a state-approved school of professional nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two years of nursing experience, including one year in home care or a closely related field
- One year of supervisory and/or case management experience preferred
- Current CPR certification
