About the Position:

This is a full-time opportunity offering a dynamic work environment and opportunities for professional growth. As a Hospice Nurse, you will be responsible for coordinating all hospice services in assigned counties, collaborating with healthcare professionals to ensure seamless patient care and supporting patients and their families through end-of-life care.

Responsibilities:

- Coordinate patient care activities, including assessments, goal setting, and outcome evaluation.
- Develop and maintain accurate records, reports, and documentation to meet regulatory requirements.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes.

Requirements:

- Active RN licensure in North Carolina (or willingness to obtain).
- Minimum one year of experience in hospice or palliative care settings.
- Current CPR certification and ability to work flexible hours.
