Join us at Positive Impact Health Centers INC as a Community Health Worker-CTL As a key member of our team, you will provide on-site and offsite brief HIV pre/post-test counseling and rapid HIV testing to clients. This is an exciting opportunity to make a positive impact in the lives of individuals affected by HIV/AIDS.
Responsibilities:The Community Health Worker-CTL will work closely with the Senior Prevention Program Manager and Director of Prevention to ensure that clients have access to medical, pharmacy, dental, behavioral health, and social services. Key responsibilities include:
- Conducting HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling standards.
- Assisting with follow-up to ensure that persons diagnosed as living with HIV are successfully linked to care and partner services.
- Providing effective condom education and distributing condoms to target HIV-infected persons and persons at highest risk of acquiring HIV.
