Key Responsibilities
  • Provide direct patient care and assume responsibility for patients in your panel.
  • Supervise trainees providing direct care to patients and provide high-quality teaching and clinical care.
  • Oversee clinical panel management and collaborate with the Nurse Care Manager to care for high-risk patients.
  • Work with the team to achieve and maintain desired patient outcomes in a cost-effective manner.
  • Develop, implement, and evaluate shared care plans for individual patients and groups of patients in collaboration with other team members.
Requirements
  • MD, DO, or equivalent degree.
  • Board-certified or board-eligible in Family Medicine, Internal Medicine, or Internal Medicine/Pediatrics.
  • California medical license or equivalent certification/permit.
  • Excellent communication and leadership skills.
  • Ability to work effectively in a multidisciplinary team environment.
