Position Overview:

The Clinical Social Services Specialist is responsible for the development, coordination, and delivery of comprehensive social services to patients and their families. This role operates within an acute care environment, focusing on partial hospitalization and intensive outpatient services.

Key Responsibilities:

Therapeutic Services:

  • Deliver therapeutic interventions to individuals, groups, families, and significant others.
  • Conduct individual therapy sessions tailored to patient needs and program requirements.
  • Provide crisis intervention and daily safety assessments.
  • Engage with patients and their support systems with respect and dignity.
  • Facilitate psychoeducational support sessions for patients and families.
  • Manage casework and therapeutic strategies for all assigned patients.
  • Act as an advocate for patients in addressing grievances in accordance with established policies.
  • Maintain effective communication with referring providers and care teams.
  • Implement individualized goals and interventions as outlined in the Care Plan.
  • Ensure that goals are SMART (specific, measurable, attainable, relevant, and timely).
  • Document patient progress in alignment with treatment plans.

Documentation Responsibilities:

  • Complete all necessary documentation in a timely manner, including admissions assessments and treatment plans.
  • Conduct suicide risk assessments and collaborate with the treatment team as per guidelines.
  • Prepare safety plans and obtain consent for record releases as needed.
  • Document ongoing treatment progress and patient engagement in group settings.
  • Summarize patient progress during multidisciplinary team meetings.

Discharge Planning:

  • Develop and document initial discharge plans and summaries.
  • Coordinate with outpatient providers and stakeholders for effective aftercare transitions.
  • Report and document any suspected cases of abuse in accordance with legal and policy requirements.

Leadership and Clinical Guidance:

  • Evaluate interdisciplinary interventions and adjust Care Plans as necessary.
  • Participate in treatment planning and progress reviews.

Professional Communication:

  • Collaborate with other departments and agencies as required.
  • Communicate critical information to the treatment team and engage in problem resolution.
  • Attend departmental meetings and contribute to discussions on clinical matters.

Qualifications:

  • Master's Degree in Psychology, Social Work, or a related field from an accredited institution.
  • Full licensure as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).
  • Knowledge of therapeutic techniques applicable to diverse age groups and understanding of human behavior dynamics.
  • Completion of required certifications and training within specified timeframes.
