About the Job

We are seeking a compassionate and skilled RN Staff Nurse to join our Hospice team at Trinity Health. As a Hospice Nurse, you will provide high-quality patient care in an atmosphere sensitive to each person's physical, emotional, social, and spiritual needs.

Responsibilities
  • Assess and evaluate patient care for an assigned group of patients, utilizing evidence-based practice.
  • Collaborate with patients and families to develop and implement a plan of care that includes physical, spiritual, and educational components.
  • Accurately communicate pertinent information to appropriate team members in a timely manner.
  • Document accurately and completely pertinent assessments, interventions, and outcomes for patients in accordance with documentation standards, policies, procedures, and/or guidelines.
  • Assume responsibility and accountability for care provided and documented by other licensed and unlicensed caregivers as appropriate.
  • Utilize technology appropriately and demonstrate continuous learning.
  • Assist in the orientation of new staff and support students' experiences as appropriate.
  • Maintain current knowledge, skills, education, and licensure in clinical practice, attend required inservices and staff meetings, and demonstrate professional growth.
  • Participate in a performance improvement approach that includes both intradepartmental and interdepartmental activities.
Requirements
  • Minimum of 2 years' work experience as a professional nurse.
  • Ability to operate a motor vehicle required; possesses a valid driver's license from the State of Ohio and has access to a functional automobile during work hours.
  • Requires use of a personal vehicle with valid auto insurance.
  • Current clinical experience, preferably in community home health or hospice setting.
  • Criminal background check and fingerprinting completed as required by Ohio law.
  • Graduate from a school of nursing. Baccalaureate degree in nursing preferred.
  • Current license to practice in the state of Ohio.
