Position OverviewUnder the guidance of the Director of Residential Care, the Patient Care Assistant (PCA) delivers direct support and oversight to residents, while observing and reporting on their conditions to the Individual Service Plan Team.
Key Responsibilities
1. Provide compassionate and secure care for all residents.
2. Respond promptly and professionally to clients' needs with courtesy.
3. Deliver care and supervision to residents as outlined in their Individual Service Plan, including monitoring vital signs and recording/reporting any signs or symptoms of illness.
4. Recognize when assistance or medical intervention is necessary and take appropriate action.
5. Monitor residents' vital signs, and contact the physician or emergency services as required.
6. Adhere to all organizational policies and procedures, and participate in staff meetings, training, and workshops.
7. Ensure residents take their medications as prescribed, monitor weight changes, and report significant variations to the Registered Nurse.
8. Complete end-of-shift reports detailing resident conditions and house activities for the incoming shift and management staff.
9. Conduct regular rounds of the house at least every two hours, or more frequently if needed.
10. Maintain confidentiality of resident information at all times.
11. Foster positive working relationships with residents, staff, volunteers, and representatives from other agencies.
12. Exhibit self-direction and initiative in prioritizing, organizing, and fulfilling job responsibilities.
13. Assist residents with housekeeping and laundry as necessary, ensuring that living spaces are tidy and free of hazards.
14. Answer phone calls in a courteous and professional manner, taking accurate messages and relaying them promptly to residents.
15. Perform additional duties as required.
This role requires the ability to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis, with the capacity to lift up to 50 lbs. Employees must be vaccinated against COVID-19 or possess a medically or religiously approved exemption.
Minimum Qualifications
1. High School diploma or equivalent.
2. Certification as a Home Health Aide or Nursing Attendant.
3. Basic knowledge of bloodborne pathogens and universal precautions, including the use of protective equipment.
4. Successful completion of a criminal background check.
5. BCLS and Red Cross Certification.
6. Bilingual proficiency in English and Spanish is preferred.
1. Must demonstrate cultural sensitivity and a non-judgmental approach.
2. Awareness and sensitivity to issues related to homelessness and/or HIV/AIDS.
3. Basic understanding of the care needs of individuals with HIV.
4. Ability to work effectively with diverse populations in a caring and sensitive manner.
5. Proficiency in reading, writing, speaking, and understanding English and Spanish.
6. CPR/First Aid Certification.
