Job Title: Intake Coordinator

Carle Foundation Hospital is seeking a skilled Intake Coordinator to join our team. As an Intake Coordinator, you will play a critical role in ensuring the timely and efficient transmission of referrals to our operational areas.

Key Responsibilities:
  • Collect demographic and clinical data to support admission into home health programs
  • Compile referral statistics and other reports as requested by leaders
  • Develop fundamental knowledge of diagnosis coding and make accurate recommendations to field staff
  • Identify patient situations and conditions that may limit or preclude successful admission to home health program
  • Perform basic scheduling functions, if requested, during periods of staffing shortages
  • Perform general office administrative work such as filing, ordering supplies, keeping inventory, and distributing mail
Requirements:
  • Strong computer skills and knowledge of Medical terminology
  • Prefer knowledge of home health, hospice, home infusion, case management, Medicare, Medicaid, accreditation standards, and experience in dealing with third-party payers
