Key Responsibilities:
- Investigate and develop community-based resources for clients to ensure they receive the best possible support.
- Develop and monitor client Service Plans (NJISP) to ensure effective and efficient support delivery.
- Engage in regular contact with clients, including monthly, quarterly, and annual check-ins, to ensure their needs are being met.
- Provide accurate and timely documentation related to billing and client support.
- Maintain strong relationships with external agencies providing direct services to clients on our caseload.
- Utilize electronic records systems, including irecord and internal agency databases, to ensure accurate and up-to-date client information.

Qualifications:
- Bachelor's degree and 1 year of experience working with adult individuals with developmental disabilities.
- Fluency in both English and Spanish.
- Experience as a Support Coordinator or in a related field.
- Ability to pass a criminal background check, registry check, and drug screen.
- Clear tuberculosis screening.
- A valid driver's license with no more than 5 points or DUIs in the last 3 years.
- Ability to pass and maintain required trainings, such as CPR/First Aid, Medication Administration, and Positive Behavioral Support Training.
Training and Development:
- All employees are required to complete 10-15 trainings upon hire and on an ongoing basis.
- Trainings are a combination of in-person and remote sessions, utilizing technology to facilitate learning and development.
