We are seeking a compassionate and motivated Palliative Care Advanced Practice Provider to join our team at St. Luke's Health The Woodlands Hospital.
The Palliative Care Program aims to meet the palliation and enrichment needs of patients and their loved ones during times of heightened need.
This position will serve in the inpatient setting, working collaboratively with physicians and nursing staff to develop a care model that prioritizes the well-being of our patients.
Responsibilities- Develop and implement a care model that addresses the physical, emotional, and spiritual needs of patients and their families.
- Collaborate with interdisciplinary teams to provide comprehensive care and support.
- Conduct thorough assessments and develop individualized care plans.
- Provide education and support to patients and their families to promote self-care and empowerment.
To be considered for this role, you must have a current license as a PA or NP to practice in the state of Texas.
We offer a competitive salary of $60.76 per hour and a comprehensive benefits package.
