Job Summary

We are seeking a skilled Care Coordinator RN to join our team. As a Care Coordinator RN, you will be responsible for coordinating, implementing, and evaluating the home care needs of patients. You will work closely with patients, families, and healthcare professionals to ensure seamless care delivery.

Responsibilities
  • Provide a full range of advanced clinical home care services, including patient assessment, medication management, and patient/family education.
  • Assume ongoing primary responsibility for coordinating, implementing, and continually evaluating the home care needs of patients.
  • Review patient insurance information and coordinate activities involved in each individual care plan.
  • Share pertinent findings with family and professional caregivers.
Qualifications
  • Current NYS RN (Registered Nurse) license and registration required.
  • Bachelor's degree in nursing preferred; Associate's degree required.
  • Minimum of one year acute medical-surgical nursing experience required.
  • Certified Home Health Agency (CHHA) experience preferred.
  • Bilingual English and Spanish or Mandarin or Cantonese preferred.
