Key Responsibilities- Assess patient responses to drug therapy and develop personalized treatment plans in collaboration with physicians.
- Order and evaluate laboratory tests to monitor drug therapy effects and outcomes.
- Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states, working closely with medical staff.
- Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
- Administer prescription or non-prescription drugs according to established agreements or protocols.
- Evaluate medical and medication histories to identify drug-related problems and adjust therapy accordingly.
- Consult with other healthcare providers to discuss patient treatment needs or options.
- Perform a full range of pharmacist procedures, including reviewing patient profiles and ensuring accuracy of pharmaceutical products.
- Provide medication and healthcare-related in-service training to clinical staff on new medications, appropriate use, and other topics as deemed necessary.
- Document significant medication interactions and pharmacy interactions with prescribers, and conduct Adverse Drug Reaction (ADR) reporting as required.
- Facilitate medical staff drug utilization reviews (DURs) as needed.
- Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited institution.
- Completion of a clinical pharmacy residency or fellowship, or documentation of equivalent education, training, and/or CME in clinical pharmacy.
- Basic Life Support certification through the American Red Cross or American Heart Association.
- One year of hospital-based pharmacy experience in unit-dose distribution and IV additives, or completion of a hospital residency.
- Current, full, active, and unrestricted license to practice as a Pharmacist.
- US citizenship.
