Job Summary:

Granite Rehabilitation and Wellness is seeking a skilled Clinical Case Manager - RN to join our team. As a key member of our interdisciplinary team, you will be responsible for conducting and coordinating the development and completion of the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plan, in accordance with state and federal requirements.

Key Responsibilities:
  • Coordinate the interdisciplinary team in the timely completion of assessments.
  • Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
  • Communicate ARD selection for specific MDSs to the interdisciplinary team.
  • Complete Care Area Assessments (CAAs) as required.
  • Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
  • Monitor the overall documentation in the medical record to validate that it supports MDS coding.
  • Participate in Medicare utilization management as assigned.
Requirements:
  • Current Registered Nursing license in the state of employment.
  • Computer aptitude with the EMR for this position.
  • Preferred: Two years' experience working in a long-term care setting.
  • Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification.
  • Preferred: Point Click Care (PCC) experience.
