Job Summary

We are seeking a highly skilled and experienced RN to lead our Home Telehealth Case Management team. As a Lead RN, you will be responsible for overseeing the daily operations of the connected care nursing program, providing programmatic, educational, and clinical support, and collaborating with interdisciplinary teams to improve patient care and outcomes.

Responsibilities
  • Provide oversight to RPM-HT staff in the daily operations of the connected care nursing program.
  • Develop and implement programmatic, educational, and clinical strategies to improve patient care and outcomes.
  • Collaborate with interdisciplinary teams to identify improvement opportunities and enhance staff education and professional development.
  • Lead improvement initiatives to enhance patient care, identify improvement opportunities, and enhance staff education and professional development.
  • Provide direct care utilizing all aspects of the nursing process, care management principles, and technologies within a collaborative, interdisciplinary practice setting.
  • Develop and implement evidence-based practice guidelines to improve patient care and outcomes.
  • Collaborate with local, VISN, and national workgroups to improve organizational performance metrics and implement educational plans.
Requirements
  • One year of Remote Patient Monitoring-Home Telehealth (RPM-HT) RN Case Management experience.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth.
  • English Language Proficiency.
