About the Role

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Horizon Services. As a Care Coordinator, you will play a vital role in providing exceptional care and support to our clients, helping them navigate the complexities of the healthcare system and achieve their goals.

Key Responsibilities
  • Conduct comprehensive assessments of clients' needs and develop personalized care plans to address their unique challenges and goals.
  • Manage a caseload of clients, providing regular check-ins, support, and guidance to ensure they receive the care and services they need.
  • Collaborate with clients' care teams, including healthcare providers, family members, and other stakeholders, to ensure seamless care coordination and effective communication.
  • Connect clients with community resources and services, helping them access the support and care they need to thrive.
  • Develop and maintain strong relationships with clients, their families, and other stakeholders, fostering trust and confidence in our services.

Requirements
  • Bachelor's degree in a human services-related field or equivalent experience.
  • At least 6 months of experience in care coordination or a related field.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
Location and Hours

This position is located at our Tonawanda office, with regular travel to meet with clients in their homes or in the community. The typical schedule is Monday through Friday, 8:00 AM to 5:00 PM, with flexibility to accommodate client needs and team requirements.
