Chsbuffalo is seeking a skilled Registered Nurse IV to join our Home Care team in Niagara County.
Job Summary:The Registered Nurse IV will provide professional nursing care to patients in their homes, utilizing infusion therapy skills and knowledge to deliver high-quality patient care. The ideal candidate will possess strong assessment skills, excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities:- Provide infusion therapy to patients in their homes, utilizing PICC lines and central catheters.
- Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
- Collaborate with healthcare teams to coordinate patient care and ensure seamless transitions between settings.
- Document patient information and progress in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate from an accredited school of nursing, college or university.
- Licensure and current registration as a Registered Professional Nurse in the State of New York required.
- BSN preferred.
- BLS certification preferred.
- PICC Certification preferred.
- Minimum one (1) year relevant experience required.
- One (1) year experience performing veni-puncture and infusion therapy.
- One (1) year experience with central catheters and PICC lines.
- Certified Home Care experience preferred.
- Critical care experience preferred.
- May be exposed to unsanitary conditions in some home settings.
- May be exposed to high crime areas within the community, within the county serviced by the agency.
- Endures exposure to weather and temperature extremes.
