About the Role

Horizon Health Services is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in supporting individuals in our community with mental health and addiction challenges.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans to address member needs and strengths.
  • Manage a caseload of approximately 40 members, providing ongoing support and guidance.
  • Travel to meet members in their homes or in the community, building strong relationships and trust.
  • Connect members with local resources and services, ensuring they receive the support they need.
  • Collaborate with care teams to ensure seamless delivery of services.
  • Creative problem-solving to overcome practical obstacles and meet member needs.
  • Embody Horizon's values of integrity, respect, and passion, always striving to do the right thing.

Requirements:

  • Bachelor's Degree in a Human Services-related field or Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification.
  • At least 6 months of experience in care coordination or behavioral health.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Organizational, time management, and problem-solving skills.
  • A clean, valid NYS driver's license and reliable transportation.

Location and Schedule:

This position is located at 1000 Young Street, Tonawanda, New York. The schedule is Monday through Friday, 8:00 AM - 5:00 PM, with flexibility to accommodate your needs.
