Compassionate Social Worker for Seniors

As a Social Worker, you will play a vital role in enhancing the well-being and quality of life for our residents living with memory loss. You will work closely with residents, families, and interdisciplinary teams to provide person-centered care that prioritizes individual needs and goals.

Responsibilities:
  • Provide comprehensive support to each resident, helping them achieve their highest level of well-being.
  • Conduct thorough assessments, including pre-admission evaluations, memory screenings, and ongoing daily assessments.
  • Orient new residents and families to our specialized care unit, offering continuous guidance and support throughout their journey.
  • Develop and implement individualized care plans for residents with severe memory loss, collaborating effectively with the interdisciplinary team.
  • Monitor resident well-being closely and make recommendations regarding levels of care as needed.
  • Empower residents to pursue their life goals and aspirations.
  • Co-facilitate a supportive memory support group for residents and their families.
Qualifications:
  • Bachelor's degree in Social Work (BSW) or a related field is required; Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
  • Experience working with older adults and individuals living with dementia is highly valued.
  • Exceptional interpersonal and communication skills, demonstrating professionalism, sensitivity, and compassion in all interactions.
  • Ability to manage challenging conversations with empathy and understanding.
  • Strong computer proficiency and time management skills are essential.
  • Highly detail-oriented and organized, with the ability to multitask effectively and meet deadlines consistently.
