We are seeking a skilled Admissions RN to join our team at Providence Service. As a key member of our hospice care team, you will be responsible for providing direct health care services to patients and their families in their homes.
Key Responsibilities- Provide independent clinical expertise throughout the nursing process
- Travel to see patients within Thurston, Mason, and/or Lewis counties
- Handle admissions for Hospice patients
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Communicate effectively with patients, families, and healthcare teams
- Graduate of a school of nursing
- Washington Registered Nurse License
- Provider BLS - American Heart Association within 30 days of hire
- 2 years of Nursing experience
- 1 year of Hospice, home health, skilled nursing facility, medical/surgical, or oncology experience
