Public Health Nurse

We are seeking a highly skilled Public Health Nurse to manage our Community Health Program. The successful candidate will be responsible for overseeing the daily management of the program, providing a full range of public health nursing services to the community, and working closely with our team to achieve our mission.
Key Responsibilities
- Plan, conduct, and evaluate a comprehensive community health nursing program to align with our mission statement.
- Assess and diagnose needs, and set up a plan of action to meet these needs by providing health services to families and individuals.
- Provide primary care for acute and chronic health problems in collaboration with medical clinic staff and homecare programs.
- Direct all activities of medical and safety Community Health Representatives (CHRs) and van driver MTD, including organization and documentation of their activities.
- Plan and participate in Programs for Diabetes Management and Primary Prevention.
- Plan and participate in Programs for Infectious Disease Control.
- Provide in-home healthcare to all populations from pre-natal care to geriatric care.
- Provide skilled nursing services to individuals and families, including measures to promote wellness, to prevent complications and to minimize disabilities.
- Teach recognition of early symptoms of communicable diseases and the importance of immunizations.
- Update immunization records and establish a notification to patients that immunizations are required.
- Give immunizations to patients who come into the clinic specifically for their vaccines and/or immunizations as prescribed by the Public Health Nurse PHN.
- Record all immunizations given in the clinic onto the NMIIS statewide system as appropriate.
- Conduct epidemiological and community surveys, field investigations and research studies to appraise health needs.
- Participate in continuing quality improvement activities for the Public Health Nursing program.
- Place referrals for routine care: mammography, colonoscopies, referral renewals, etc.
- Maintain records and reporting systems according to Sandia Health Center PSHC policies for patients.
- Work with the Tribe, State and Federal Health Agencies, in planning health related activities.
- Employees will adhere to all applicable Pueblo of Sandia Human Resources Team Member Handbook Employee Manual policies and procedures and other PSHC Health Center Administrative policies and procedures.
- Punctual and regular attendance is an essential responsibility for all team members and are expected to report to work based upon the assigned schedule each day in accordance with the Attendance Policy for Pueblo of Sandia.
- May be crossed trained and act as covering PSHC Registered Nurse, or medical and/or safety CHR.
- Demonstrates strong interpersonal skills in building a cohesive working relationship with co-workers and internal/external contact.
- Performs other job-related duties as assigned.
Education and Experience
- High School Diploma, GED certification or equivalent.
- Associates degree (BSN) in nursing from an accredited college or university.
- Three (3) years' experience in public nursing including two (2) years supervisory experience.
- Current RN Licensure with the NM Board of Nursing.
- Current CPR and First Aid certifications.
- The following immunizations and titers are required prior to employment start date:
- Hepatitis B series or immune by titer.
- Tdap.
- MMR or immune by titer.
- Annual TB test.
- Annual Flu vaccination (seasonal).
- COVID booster (optional but encouraged).
