Job Title: Nurse Practitioner - Homeless Health Specialist

Albuquerque Health Care for the Homeless is seeking a compassionate and skilled Nurse Practitioner to join our team. As a Homeless Health Specialist, you will provide high-quality, patient-centered care to individuals experiencing homelessness.

Key Responsibilities:
  • Provide primary care services to homeless individuals, including chronic disease management and substance use disorder treatment.
  • Work collaboratively with our mobile teams to provide on-site treatment in outreach settings.
  • Embodiment of harm reduction philosophies and trauma-informed care principles.
  • Comfortable working in a shared client model with multiple practitioners.
  • Participate in ongoing education and training to enhance skills and knowledge.
Requirements:
  • Master's degree in Nursing.
  • CNP license through the NM Board of Nursing.
  • Current DEA license.
  • Current CSR license.
Preferred Qualifications:
  • 2 years of experience providing medical care in primary or family practice setting.
  • 2 years of experience in Federally Qualified Health Center or in public health.
  • Experience treating co-morbid conditions.
  • Experience with eCW EHR.
  • Knowledge of harm reduction and trauma-informed philosophies.
  • Previous work with under-served populations or people experiencing homelessness.
