We offer a wide range of advanced medical, surgical, and specialty care services that allow our urban campus in downtown Grand Rapids to serve as a major tertiary and quaternary referral center for West Michigan and beyond. These services include the only Regional Burn Center and Level I Trauma Center in West Michigan.
Job SummaryMaintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations.
Essential Functions- Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
- Safely compounds and dispenses medications for administration to patients.
- Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles.
- Provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
- Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
- Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
- Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
- Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
- Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
- Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy.
- 2 years of relevant experience hospital pharmacy Preferred.
- American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred.
- LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required.
- LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required.
- CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required.
