Job SummaryMaintain accountability for ensuring the safety and efficacy of medication use. Educate and act as a consultant to patients/family and members of the health care team related to optimal drug therapy and participate in process improvement activities and drug use evaluations.
Key Responsibilities- Interpret and screen medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
- Safely compound and dispense medications for administration to patients.
- Evaluate patients, order appropriate laboratory tests, and interpret serum drug levels utilizing pharmacokinetic principles.
- Provide prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
- Provide appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
- Engage in continuous study of the entire professional field to maintain professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
- Required: Bachelor's Degree in Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy.
- 2 years of relevant experience in hospital pharmacy.
- Preferred: American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice.
- Required licenses and certifications: LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days, LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days, CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days, or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days, or CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days.
The Pharmacy Department provides pharmacy services for Inpatients, ED, Surgery, and the Infusion Center at Corewell Health Butterworth Hospital.
Work EnvironmentEvening shift, 2:30 p.m. to 11:30 p.m., 7 days on/7 days off, every other weekend.
