Key Responsibilities
  • Coordinate health care and medical-related case management for clients
  • Conduct basic health assessments, check vitals, blood pressure, and weight
  • Provide ongoing health monitoring to manage and support clients' health needs
  • Make appropriate referrals and follow-up for medical, substance use, and/or psychiatric treatment
  • Observe, monitor, and document client self-management of medication
  • Process physicians' orders and obtain prescriptions
  • Collaborate with medical and psychiatric providers and hospitals
  • Develop and facilitate health-related educational groups and one-on-one client education
  • Participate in interdisciplinary team meetings and case conferences
  • Respond to medical emergencies and provide emergency first aid/CPR, and Narcan opioid overdose reversal when needed
Requirements
  • NYS LPN license required
  • Bachelor's Degree preferred
  • Experience working with individuals experiencing homelessness preferred
  • Knowledge of mental illness and substance use disorder helpful
  • Proficiency in MS Office, including Word and Excel
  • Bilingual skills a plus
  • CPR training certification or willingness to complete certification training in CPR and opioid overdose prevention
