The Healthcare Team Supervisor is tasked with overseeing, guiding, coordinating, assessing, and enhancing client care teams to guarantee high-quality service delivery and effective case management within the operational framework of Brooks Rehabilitation's offerings and regulations; as well as adhering to state, federal, and local statutes; and the Nurse Practice Act. This role includes the supervision of staff and all patient care services rendered. The Supervisor may occasionally engage in direct patient care by applying the nursing process and recognized standards of practice. Actively participates as a key member of the patient's interdisciplinary treatment team. The Supervisor also directly manages the patient care team and aids in executing the patient's care plan.

Key Responsibilities:

Must be a Registered Nurse. Directs and manages a team of Nurse and Therapist Case Managers to ensure safe, effective, and suitable home care services. Remains accessible during standard business hours and as necessary to provide support and ensure quality care delivery to home care clients.

Facilitates the coordination and reception of referrals, assesses required services, and evaluates Brooks Rehabilitation's capacity to meet client needs. Assigns patient and personnel responsibilities, coordinates patient care, and regularly meets with Case Managers and clinical staff to offer guidance and address specific issues.

Ensures the creation, execution, and updates of individualized care plans. Provides direction to teams to ensure that client needs are met and services are delivered in accordance with the care plan and current clinical practice guidelines and professional standards.

Assists clinical staff in prioritizing tasks, setting objectives, and evaluating progress towards those objectives. Leads case conferences and other clinical meetings to facilitate care coordination and ensures continuous assessment of patient needs. Collaborates with Brooks Rehabilitation Leadership to identify departmental budgetary needs and priorities. Reviews weekly productivity reports of team members. Coordinates 24-hour Agency coverage by Registered Nurses and evaluates the quality of on-call services. Provides on-call coverage as required.

Delivers direct patient care in the field in line with RN job specifications. Develops collaborative relationships with other healthcare professionals in the community. Provides support and guidance to Agency staff, other healthcare professionals, clients, and families regarding available healthcare resources. Educates Agency staff on clinical services, policies, and procedures as necessary. Facilitates problem-solving sessions to assist Case Managers and other staff in resolving client and/or reimbursement source issues. Remains informed about community resources, healthcare costs, and industry trends through self-education and access to external educational opportunities. Ensures the accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies, regulatory requirements, and industry standards. Regularly monitors open and closed charts, participates in quarterly clinical record reviews, and engages in Quality Assessment and Performance Improvement (QAPI) activities. Reviews Agency policies and procedures and recommends necessary changes or revisions. Provides educational programs and information regarding appropriate documentation practices. Offers leadership to the team and support staff in identifying Agency/client needs and opportunities for quality enhancement.

Assists with marketing, public relations, and discharge planning by participating in departmental meetings. Aids quality improvement teams with data collection for the Quality Assessment and Performance Improvement Plan established by Brooks Rehabilitation. Reviews OASIS data collection to ensure accuracy and compliance with requirements. Communicates with clinicians and provides training as needed to achieve compliance with collection and reporting timelines. Oversees clinical outcomes and process measures of teams. Actively implements strategies to enhance outcomes, including those identified in the QAPI program. Identifies and enacts changes in clinical and/or operational practices based on QAPI findings. Interprets and enforces human resource policies and procedures consistently and fairly. Assists in the recruitment and interviewing of new Agency personnel and makes recommendations to the Director of Nursing. Aids in the orientation of new staff. Conducts timely performance evaluations in accordance with Agency policy. Provides on-site supervision as needed to assess staff competency and address educational and developmental needs. Adheres to Agency guidelines for disciplinary actions and documents all actions in accordance with Human Resource policies and legal standards. Monitors employee turnover, overtime, and absenteeism, taking appropriate actions to address issues. Collaborates with Agency Director of Nursing and other clinical management staff to ensure proper staffing of qualified personnel. Promotes personal safety and a secure environment for clients and colleagues. Demonstrates knowledge of safety/infection control practices by complying with policies and regulatory requirements. Assesses the safety of the environment and proactively helps prevent accidents and promote safety. Recognizes and responds appropriately to potentially unsafe situations. Performs additional duties as assigned. Maintains a working knowledge of commonly used medical equipment/devices. Respects confidentiality of client clinical records and shares information only in accordance with Agency policy and HIPAA regulations. Educates patients and families on medications, conditions, and services. Ensures adherence to clinical practice standards at all times and oversees the development, implementation, and updates of individualized care plans. Acts professionally, adheres to scheduled work hours, and arrives on time.

Collaborates with the Director of Nursing to ensure compliance with Medicare guidelines and revisions. Follows procedures to operate within legal frameworks. Keeps the Director of Nursing or Administrator informed of any issues or potential concerns. Meets with nurses, therapists, home health aides, and other disciplines involved in patient care for weekly case conferences. Takes and documents physician orders as necessary and appropriate. Establishes and maintains ongoing relationships with physicians and their office staff, facilities, or other community members involved to promote continuity of care. Represents Brooks Rehabilitation professionally and loyally at all times. Attends meetings, luncheons, and in-services as required. Provides guidance to clinicians and staff as needed. May perform other duties as assigned.

Qualifications:

  • Graduate of an accredited nursing program; BSN preferred.
  • Registered Nurse with a current/valid Florida Nursing license and approved Hands-On CPR certification.
  • A minimum of two years of experience as a registered professional nurse and one year of supervisory experience preferred, ideally in home health care.
  • Demonstrates organizational and leadership capabilities.
  • Possesses a working knowledge of Medicare guidelines for home health coverage and compliance.
  • Exhibits strong verbal and written communication skills, evaluation techniques, and demonstrates knowledge of clinical skills, judgment, current nursing practices, and analytical abilities.
  • Familiarity with reimbursement sources and documentation requirements within home health care.
  • Knowledge of Medicare, Medicaid, and other third-party payer guidelines.
  • Must hold a current/valid driver's license, agency-required automobile liability insurance, and have access to reliable transportation for home visits. Capable of assessing patient status and identifying requirements relative to age-specific needs.
  • Participates in the development of agency policies.
  • Ensures care is delivered in accordance with current clinical practice guidelines and quality outcomes.
  • Current CPR certification.
  • Hands-On BLS certification.
