Key Responsibilities
  • Conduct Comprehensive Assessments: Perform thorough and ongoing evaluations of patient needs, identifying areas for improvement and developing personalized care plans to achieve optimal outcomes.
  • Lead Care Teams: Coordinate and manage care teams, ensuring seamless communication and collaboration among healthcare professionals to deliver exceptional patient care.
  • Develop and Implement Care Plans: Create and implement comprehensive care plans that address patient needs, incorporating evidence-based practices and best practices in home health care.
  • Monitor and Evaluate Patient Progress: Regularly assess patient progress, making adjustments to care plans as needed to ensure patients achieve their goals and maintain optimal health.
  • Collaborate with Healthcare Professionals: Work closely with physicians, nurses, therapists, and other healthcare professionals to ensure comprehensive and coordinated care.
  • Ensure Regulatory Compliance: Maintain knowledge of and adhere to relevant laws, regulations, and industry standards, ensuring CenterWell Home Health remains compliant and up-to-date.
Requirements
  • Education: Bachelor of Science degree in Nursing (BSN) or related field.
  • Experience: At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Certifications: Current CPR certification and valid driver's license.
  • Skills: Excellent verbal and written communication skills, strong interpersonal skills, and ability to work effectively in a fast-paced environment.
