Job Title: Assistant MDS Coordinator

The Terrace of St. Cloud is seeking a highly skilled and detail-oriented Assistant MDS Coordinator to join our team. As an Assistant MDS Coordinator, you will play a critical role in ensuring the accuracy and completeness of Minimum Data Sets (MDS) and associated processes.

Key Responsibilities:
  • Coordinate the completion of MDS and Plan of Care in a timely and accurate manner.
  • Develop and implement schedules for MDS completion, ensuring compliance with state and federal regulations.
  • Collaborate with the interdisciplinary team to monitor quality indicators and identify areas for improvement.
  • Provide education and training to staff on MDS-related topics, including ADL coding and skilled documentation.
  • Assist in the development and implementation of care plans, ensuring resident-centered care.
  • Participate in quality assurance and performance improvement initiatives.
  • Maintain accurate and up-to-date records, including MDS and care plans.
Requirements:
  • Minimum of 3 years of experience as an MDS Coordinator.
  • 3-5 years of experience working in a Long Term Care Center.
  • Current, unencumbered RN license in the state of employment.
  • Eligible AHCA Background Screening Status.
  • Knowledge of rehabilitation and restorative care, as well as computer proficiency.
  • Experience with computerized MDS systems and current MDS/RAI guidelines.
