About the Role

We are seeking a highly skilled and experienced MDS Coordinator to join our team at The Terrace of St. Cloud. As a key member of our nursing department, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Coordinate the completion of Minimum Data Sets (MDS) and Plan of Care in a timely and accurate manner, adhering to state and federal regulations.
  • Develop and implement schedules for MDS completion, ensuring seamless coordination with interdisciplinary teams.
  • Monitor quality indicators to identify trends and areas for improvement, contributing to the enhancement of resident care.
  • Collaborate with clinical teams to ensure accurate and comprehensive MDS documentation.
  • Provide education and support to staff on MDS-related topics, including ADL coding and skilled documentation.
  • Participate in committees and action plans to further develop the care planning process, ensuring alignment with facility goals.
  • Ensure compliance with state and federal standards and regulations, maintaining a high level of resident care and satisfaction.
Requirements
  • Minimum of three years of experience as an MDS Coordinator in a long-term care setting.
  • Current, unencumbered RN license in the state of employment.
  • Eligible AHCA Background Screening Status.
  • Knowledge of rehabilitation and restorative care, as well as computer proficiency and experience with MDS systems.
  • Effective communication, interpersonal, and organizational skills, with the ability to work independently and as part of a team.
