We are seeking a skilled MDS Coordinator to join our team
Job SummaryThe MDS Coordinator will be responsible for maintaining and following a schedule of due dates for all MDS, coordinating the completion of the MDS by all disciplines, and reviewing the MDS data for accuracy.
Key Responsibilities- Maintain and follow a schedule of due dates for all MDS
- Coordinate the completion of the MDS by all disciplines
- Review the MDS data for accuracy and meet with appropriate staff as needed to assure accuracy
- Complete the MDS and input into the computer
- Verify that assessments have been transmitted and approved by the State in a timely manner
- Correct any rejected records and prepare them for re-submission
- Complete CAAs according to state guidelines
- Formulate Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame
- Schedule and conduct RCP meetings on a regular and timely basis
- Communicate the RCP to appropriate staff members and notify supervisor if noncompliance by staff is noted
- Work with all in-house and ancillary departments to assure understanding and compliance with the RCP
- Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year
- Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements
- Experience with MDS-RAP and Care Planning functions
- Prefer experience with RUG-IV
- Prefer experience with MDS 3.0
