Case Manager

We are seeking a dedicated and compassionate Case Manager to join our Substance Use Disorder (SUD) Clinic team at Armada Recovery. As a Case Manager, you will play a vital role in providing comprehensive support and assistance to individuals seeking treatment for substance use disorders in the community.

Key Responsibilities:
  • Assessment and Planning: Conduct thorough assessments of clients' needs, including substance use history, mental health status, and social determinants of health.
  • Treatment Coordination: Collaborate with clients to develop individualized treatment plans that address their unique needs and goals.
  • Referral and Linkage: Coordinate and facilitate referrals to appropriate community resources, including detoxification services, counseling, medical care, and support groups.
  • Progress Monitoring: Monitor clients' progress throughout their treatment journey, providing ongoing support, advocacy, and crisis intervention as needed.
  • Documentation: Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress notes in accordance with clinic policies and regulatory requirements.
Requirements:
  • Ohio Medicaid ID: Valid Ohio Medicaid ID is a must.
  • Experience: Previous experience working in a substance use disorder treatment setting is highly desirable.
  • Knowledge: Knowledge of Ohio Medicaid policies and procedures.
  • Community Resources: Familiarity with community resources and referral networks for substance use disorder treatment.
  • Communication Skills: Strong communication and interpersonal skills to effectively engage and build rapport with clients from diverse backgrounds.
  • Teamwork: Ability to work independently and as part of a team in a fast-paced and dynamic environment.
  • Organizational Skills: Excellent organizational and time management skills to effectively prioritize tasks and meet deadlines.
