Job Title: Clinical Case Manager

Daybreak is seeking a highly skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a critical role in providing comprehensive clinical case management services to youth and young adults in our Youth Homelessness Demonstration Project (YHDP) Rapid Rehousing (RRH) program.

Key Responsibilities:
  • Provide individualized case management, psychoeducation, life skills education, and linkage with educational, employment, financial, housing, medical, and other social resources in the community tailored to the clients' treatment plan goals.
  • Foster life skill development, daily functioning restoration, social skills enhancement, and housing stability through personalized support, meeting clients where they are, to empower them to overcome challenges and achieve goals.
  • Develop and oversee implementation of individualized Treatment Plans with client and service team, ensuring progress towards clients' goals.
  • Deliver 2-3 services per week per client, or as specified in their treatment plan and/or discussed during supervision based on the client's barriers to permanent housing and self-sufficiency.
  • Maintain communication, collaborate, and refer to clients' service team such as employment, engagement, parenting, substance abuse treatment, and life enrichment individual/group activities to ensure clients advance their treatment plan goals.
Requirements:
  • Bachelor's degree in social work or counseling required.
  • Licensure from the Counselor, Social Worker, and Marriage & Family Therapist Board required.
  • Valid driver's license, good driving record, and insurance required.
What We Offer:
  • Annual salary range: $60,000 - $65,000.
  • Benefits package includes health, dental, vision, life insurance, supplemental life insurance, STD & LTD, pet insurance, retirement plan, employee assistance program, 10 paid holidays, vacation accrual, and sick leave accrual.
  • Hybrid (1 day a week) and flexible work schedules.
