About the Role

We are seeking a highly motivated and experienced Family Partner to join our team at The Home for Little Wanderers. As a Family Partner, you will play a critical role in supporting families with children and youth who have special needs and mental health challenges.

Key Responsibilities
  • Work closely with families to provide guidance, coaching, education, and support throughout the care planning process.
  • Participate as a member of the Care Planning Team (CPT) and clearly outline the goals of the Family Support and Training Services.
  • Collaborate with families, behavioral health providers, and other community resources to implement the objectives and goals identified in the Individual Care Plan.
  • Conduct regular home and community visits, provide ongoing telephone support, and teach parents/caregivers how to promote linkages with other treatment providers and community resources.
  • Educate parents/caregivers about navigating child-serving systems and accessing informal/community resources.
  • Attend staff meetings, present client/family material, and participate in regular individual and group supervision.
  • Complete daily progress notes, document significant family interactions, and maintain accurate records.
Requirements
  • High school diploma or equivalent and a minimum of two years' experience working with children, adolescents, or transition-age youth.
  • Associate's degree in a human services field and one year of experience working with children, adolescents, or transition-age youth.
  • Bachelor's degree in a human services field from an accredited university and one year of experience working with children, adolescents, or transition-age youth.
  • Knowledge of the home community(ies) of the families to be served.
  • Bilingual/Bicultural candidates in Haitian Creole are strongly encouraged to apply.
