Responsibilities
  • Provide high-quality, patient-centered primary care services to patients residing in skilled nursing facilities.
  • Rounding and managing the clinical care of the population at assigned facilities.
  • Prescribing medications and treatments for residents, with timely documentation of all patient care/encounters.
  • Available for phone/secure messaging consultations with facility nursing staff.
  • On-site presence required for scheduled shifts, with flexibility within reason.
